Before starting:


A report is an incident/event that recently happened at work (or recognized as such) and requires a follow-up/to be logged in a register.

 On WEB application:

On mobile application:


1. Head to the employee's file. 


2. Click on the "Health" tab. 


3. Scroll down to the "Report" section.


4. Click on "New" in the drop-down list.


5. The fields to fill in appears and the report now has the "Open" status. Complete the information.


6. Save. 


7. If needed, fill in the "Lesion Tracking" section.


8. Save.


Sorry, the functionality is not available on the mobile application.


The "Supervisor" field is to indicate the supervisor present at the time of the accident (not necessarily a witness) and the employee's supervisor for whom you are creating the report. Leave the field empty if this doesn't apply.


If the same employee is involved in another incident, go back to the drop-down list under "Report". You can see their other open (recent) files. Click on "New" to create a new report.
